problem is solved, milter-ahead was accidentally build without berkeley
db support.

When using another cache-file type, one can see this in the log file:

[...]
May 12 15:35:13 mailsys-mgnt milter-ahead[22775]: milter-ahead/1.11.141
Copyright 2004, 2009 by Anthony Howe. All rights reserved.
May 12 15:35:13 mailsys-mgnt milter-ahead[22775]: LibSnert/1.73.17
Copyright 1996, 2010 by Anthony Howe. All rights reserved.
May 12 15:35:13 mailsys-mgnt milter-ahead[22775]: libmilter version 2 (4)
May 12 15:35:13 mailsys-mgnt milter-ahead[22775]: Built without Berkeley
DB support

When using the default cache-file type bdb, the only logged line is:

May 12 15:33:47 mailsys-mgnt milter-ahead[22392]: failed to create cache

IMO it would be very helpful to get the message "Built without Berkeley
DB support" if one want to use it...

Kind regards,
Edda
