The log entry and headers you presented below don't match (message id 
and timestamps) so I presume the headers given were for a message that 
came through later. Shame you disabled milter-date, instead replacing 
the -d option (I'm amazed you're game enough) with

	-s '' -v all

Then you would have more interesting logs.

Is your local mailer set to use the F=D flag, ie. include Date: header 
if missing? If so, then its normal that you'll see a Date header in the 
final message, but the original was probably really missing it.

In your log extract, we see the below, you'll note that the first header 
seen by milter-date is the "Recieved: ... InterScan Messaging Security" 
followed by the usual suspects, but in the headers of the message you 
see Return-Path, final Recieved header, Date, and Message-Id _ahead_ of 
the Recieved header from InterScan Messaging Security. These appear to 
have been added to the message during finally delievery and would 
indicate to me that they were not original present. For Return-Path and 
Recieved headers this is normal, but for the Date and Message-Id they 
appear to be added AFTER. You might want to use your MIMEDefang to 
capture an example or crank the sendmail LogLevel way up so as to log 
the headers sendmail sees as they come in (though I'm not certain 
LogLevel will log the headers and/or message body when set very high, 
like 99).

I'm pretty confident that original message had no Date or Message-ID 
headers.
